Reporting to the Project Coordinator, you will be joining our social prescribing team, working in GP practices across North Bristol (Gloucester Road, Lockleaze and Horfield area).
Working closely with healthcare professionals, our project addresses issues that may be causing or exacerbating health problems. You will work with individuals to identify their needs, then support them to access other organisations, services or community groups as appropriate.
