Come and join our team of Link Workers for Adults. They are a special bunch, helping to change peoples’ lives for the positive. They work alongside participants to help figure out what is important to them, and provide support to help participants towards this. You will make a real difference by listening to, empowering, and connecting our participants with services and their communities.
We have two exciting roles on offer and are looking for two fantastic people – is this you?
One of the roles works alongside GP surgeries in the HealthWest Primary Care Network group, and joins an established team working across North Bristol.
The other, a brand new role focussing on people discharged and returning home from Southmead Hospital, and reconnecting in the community.
You will be welcomed by our supportive and friendly Link Worker team who not only work in surgeries across North Bristol, but work connectively with our wider health and wellbeing teams and projects such as Wellbeing Coaching, Community Development, Neighbours Connect Southmead, and Greenway Community Care.
Our home base is the Greenway Centre, a much loved centre for the community adjacent to Badock’s Wood. We enjoy walks in the Wood, free membership to our onsite Gym and fitness classes, a discount with our onsite cafe, and a supportive staff network.
We are committed to building a staff team which represents the diverse community we work in. We encourage and welcome applications from all backgrounds. We are a Disability Confident employer. Please tell us if there are any reasonable adjustments you would like us to make when you attend for interview or start work with us.
